  • Annual Medicare Cost Report: A Medicare Cost Report (MCR) is a financial account submitted from Medicare-certified entities, such as skilled nursing facilities, hospitals or hospices, to a Medicare Administrative Contractor (MAC). Each entity updates its report at the end of its fiscal year, and MCRs publish quarterly.

  • Medicare Hospice CAP Report: Hospice Cap acts as a safeguard, ensuring hospices don't receive more than the estimated cost of traditional end-of-life care. This helps Medicare manage its budget and ensure fair access to hospice services for all Medicare beneficiaries.

  • Home Health and Hospice Medicare SIERA Report: Home Health Agencies (HHA) provide at home skilled nursing, personal care and therapeutic services. Hospices provide palliative care and alleviate the physical, emotional, social and spiritual discomforts of an individual who is experiencing the last phases of life due to the existence of a terminal disease. In addition, hospices provide supportive care for the primary care giver and the family of the hospice patient. Home health agencies and hospices submit an annual utilization report to the Office at the end of each calendar year. The report includes information on services capacity, visits, utilization, patient characteristics, and capital/equipment expenditures, and gross revenues. The documentation, including report forms, is available for each reporting year.
